Please arrive promptly for your appointment. You can save time by printing and filling out the New patient form ahead of time, or simply arrive a few minutes early and fill out a form at the clinic.
If you cannot come to an appointment we ask that you cancel online or by phone at least 24 hours in advance. Missed appointments and late cancellations will be charged $45, unless the reason for the cancellation is illness or weather. If you feel ill or may be coming down with something, or if the weather is inclement (snow, storms, etc.), then we encourage you to cancel your appointment even at the last minute at no charge.
Just a few more things:
1. If you wear pants they should be loose and easily rolled up to above the knees. This is important!
2. If you are coming in for pain in the hips, buttocks or thighs, please wear a sleeveless top if possible as the best points are often on the upper arms and shoulders.
3. Please don’t wear perfume or strong scents; some patients are sensitive to them.
4. If you need to cancel or reschedule please do so at least 24 hours in advance, either online, by email or by phone. To manage your appointment online, please find the confirmation or reminder email you received and click on “Manage your appointment.” Alternatively, call us at 847-796-0123.

IPHONE USERS: Please do not rely on your iPhone to navigate to our office; it will take you to 737 N. Milwaukee Ave. in Vernon Hills (instead of Libertyville), about ten minutes south of here. This does not seem to be a problem with Android devices.
